Complete the survey ONLINE (https://surveys.panoramaed.com/nycdoe/login):
- Click on the link above to go to the website.
- Type in f and then your child's ID number (for example: f123456789).
- Once completed, email Ms. Lam ([email protected]) with a screenshot of your confirmation of submission with your child(ren)'s name(s).
Complete the PAPER survey:
- Fill out the paper survey and put it into the provided prepaid white envelope. Make sure it is sealed.
- You or your child can drop off the sealed paper survey with Ms. Lam in Room 423.
